Responsibility for Loss or Damage Agreement *

• I have read and agree to the foundation’s Property and Equipment Responsibility Agreement, including responsibility for loss or damage.

Responsibility for Loss or Damage I accept responsibility for all foundation property released to me. I will use it only for the approved purpose, take reasonable care of it, and return it by the agreed date in the same condition, except for normal wear and tear. If the property is lost, stolen, or damaged while in my custody, I agree to notify the foundation immediately and be responsible for the reasonable cost of repair or replacement, subject to applicable law and any written terms agreed with the foundation. The foundation will document any loss or damage and provide the repair or replacement cost before payment is due.

Option 2 – Download & Print

To complete your application on paper, click this link: Property & Equipment Use Request Form to open, download, and print the three-page application. Once completed, you may scan and email your application to sistajoycommedu@gmail.com or deliver it directly to the foundation.

Option 3 – Apply in Person

Visit the Sista Joy Francis Community Education Foundation in Alexandria District, Clarendon, to collect, complete, and submit your application in person.

Join the Sister Joy Francis Community Education Foundation and help us empower communities through education, professional training, and development—membership fee: J$1,000 per month.

Applicants must be 18 years or older.

Membership fee is J$1,000 per month

Members must agree to follow the foundation’s policies and code of conduct

Membership is subject to review and approval

Membership benefits

• One complimentary facility event annually, subject to advance notice and board approval

• 10% discount on eligible facility rentals.

• Priority facility reservations.

• Access to designated foundation facilities and equipment subject to availability and approval.
